The transition from hospital to home can be frightening and stressful. Our goal is to help each person get back to where they need to be. We take your care personally, and we are proud of our caring and compassionate staff and interdisciplinary healthcare team that is available 24/7. Both private or semi-private suites are available. Accommodating spaces include a common area where visiting family members are welcome, patios, dining rooms, and a library with computer access.
Personalized care includes: three meals a day, routine medical transportation, daily housekeeping support, and activity programs that are tailored to each individual.
For six years, we have received a five-star rating with the Centers for Medicare & Medicaid Services, with six years deficiency-free. We are among only 72 communities in California, and our excellent rating lands us within the top 6% of communities in the state.
Skilled Nursing & Rehabilitation
Our on-site Skilled Nursing and Rehabilitation Center is prepared! In a warm and welcoming environment, we offer an array of rehabilitation services, short-term nursing care for individuals recovering from illness or injury, and long-term care for individuals needing extended nursing services.
Key Partnerships
Our Skilled Nursing rehabilitation services include an advanced therapy program called Consonus. This program provides a mix of clinical, concierge, and wellness programming proven to promote successful resident aging.
Our expert team is also certified in an advanced therapy program called Accelerated Care Plus (ACP), which accelerates the introduction of traditional therapies and promotes treatment for pain, rather than just pain management.
